Consent to the Processing of Personal Data for Marketing Purposes

I agree that KCK Cyklosport-Mode s.r.o., with registered office at Bartošova 348, 765 02 Otrokovice-Kvítkovice, Company ID: 18559751 (hereinafter referred to as the “Controller”), may, in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council, process the following personal data:

Name and surname,

Email address
(hereinafter referred to as “personal data”)

The Controller will process the above-mentioned data for marketing purposes, specifically for sending newsletters to the person giving this consent. This consent is granted for a period of ...... years (based on the type of products, we consider a duration of 3–5 years to be justifiable). The person providing this consent acknowledges that the Controller will process the above personal data for the duration of the consent.

The data will be processed solely by the Controller or by a party with whom the Controller has entered into a proper data processing agreement.

Consent may be withdrawn at any time, for example by sending an email to info@force.bike or a letter to the contact address of KCK Cyklosport-Mode s.r.o., Bartošova 348, 765 02 Otrokovice-Kvítkovice.

KCK Cyklosport-Mode s.r.o. informs its customers that they have the right to:

   4.1. Obtain confirmation from the Controller as to whether personal data concerning them is being processed, and if so, access such personal data and the following information: purposes of the processing, categories of personal data concerned, recipients or categories of recipients to whom the personal data has been or will be disclosed (especially recipients in third countries or international organizations), the planned period for which the personal data will be stored, or if not possible, the criteria used to determine that period, the existence of the right to request from the Controller the rectification or erasure of personal data or restriction of processing or to object to such processing, and the right to lodge a complaint with a supervisory authority.

   4.2. Request that the Controller rectify inaccurate personal data concerning them without undue delay. Taking into account the purposes of the processing, they have the right to have incomplete personal data completed, including by means of providing a supplementary statement.

   4.3. Request that the Controller erase personal data concerning them without undue delay, and the Controller shall have the obligation to erase personal data without undue delay if one of the following applies: the personal data is no longer necessary in relation to the purposes for which it was collected or otherwise processed, the customer withdraws consent and there is no other legal ground for the processing, the personal data was processed unlawfully, or the personal data must be erased to comply with a legal obligation under Union or Member State law.

   4.4. Request that the Controller restrict processing if they contest the accuracy of the personal data (for a period enabling the Controller to verify the accuracy), the processing is unlawful and they oppose the erasure and request restriction instead, the Controller no longer needs the data but they are required for the establishment, exercise, or defense of legal claims.

   4.5. Have their personal data transmitted by the Controller to another Controller upon request.

   4.6. Object to the processing of their personal data by the Controller.

   4.7. In case of doubts about whether their data is being processed in accordance with the applicable legal regulations, they may contact either the Controller or the Office for Personal Data Protection.

I agree that KCK Cyklosport-Mode s.r.o., with registered office at Bartošova 348, 765 02 Otrokovice-Kvítkovice, Company ID: 18559751 (hereinafter referred to as the “Controller”), may, in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council, process the following personal data:

Name and surname,

Address,

Telephone number,

Email address,

In the case of business persons: Company ID and VAT ID
(hereinafter referred to as “personal data”)

1. The above-mentioned data will be processed by the Controller for marketing purposes, specifically for sending newsletters to the person giving this consent. This consent is granted for a period of ...... years. The person providing this consent acknowledges that the Controller will process the above personal data for the duration of the consent.

2. The data will be processed solely by the Controller or by a party with whom the Controller has entered into a proper data processing agreement.

4. KCK Cyklosport-Mode s.r.o. informs its customers that they have the right to:
